I had exactly the same problem with BSDi/386, X11R5 and a Logitech Mouseman.
No joy at all.
Then last Saturday I picked up an old ( 1990 ) Series 9 Logitech Mouse.
Plugged it in, set Xconfig to the above and all was happynes and joy!
Looks like 'Logitech' realy means 'Logitech Series 9'...
