I've seen it only a couple of times, and I'm using a 386DX-40 with 8MB RAM and a Maxtor 7245 IDE drive. Interstingly enough, it also happend when I was using Interactive UNIX on an IDE drive. Switching to SCSI solved that problem. I don't have a SCSI drive for my NetBSD system, so I have not been able to try it.
