I built XFree86-1.3 just a few days ago under NetBSD-0.9. The file
ioctl_pc.h is part of the X386 source tree. I'm not at my machine
right now but it was something like mit/server/ddx/X386/etc/....
The make completed without any problems and I'm up and running.
The whole process is described in great detail in the various
readme files (I think most are in mit/server/ddx/X386).
