Does the device driver recognise the chip as a 16550? It should print a line such as "com1: fifo" during the kernel boot (accessible via `dmesg`). The only thing that springs to mind is the name "urt16550a". I seem to recall some weird problem with NS16550A chips, and certainly all the 550s I've ever used are NS16550AFs (note the "AF").
