Thanks to all the people who offered me suggestions.  I found my problem.
 
Oh dopey me, I had the wrong IRQ for the com port.  

Lesson 1: Never trust your notes, look at the hardware.  

Lesson 2: I'm surprised that it worked at all with the wrong IRQ.
