There are three button PS/2 mice. I have used one made by Logitech.
The IBM spec only talks about two buttons, so the middle button is
nonstandard, and implementations may vary. But if you look at the
protocol, you'll see there is really only one bit position in the
packet that makes sense as the "middle mouse button pressed bit."
