It looks like something is missing in the DOS header on the floppy.

mkfs is standard on FreeBSD :-). For DOS "formatting" of floppies i can
only offer you "mformat", that is part of the mtools package. Should
be somewhere under the packages directory for FreeBSD.
You can also use the "mdir" and "mcopy" programs from mtool to access
your DOS floppies instead of the pcfs-mount.
