This is exactly the same situation as mine!
The following is my configuration.

   HI-Tech USA ISA-486/33 w/ AMI BIOS and SiS Chipset
   8MB RAM
   Adaptech 1542B
   HP2213A 633MB SCSI Disk
   IBM DOS 5.0

I removed all cards (i.e. IDE/FDD,1P/2S/1G) except Video (Diamond
SpeedStar Plus) and 1542B from AT bus and re-installed 386BSD from the
FDD controlled by 1542B. But the situation was not changed.

Then, I tried to install to IDE disk (Conner CP30104H) and the 386BSD
was successfully booted from the hard drive.

So, I think our problems are related to disk controlers or hard disks.
