The author is Andy Tanennbaum and the software is put out by Prentice-Hall. This version is a crude one. However, it does work once you "finally" get the installation correctly done and besides, it is a great learning experience. I did e-mail the author some 2 years ago and he informed me that an improved version will be coming out in a couple of years -- the one I have will only boot from floppy disks -- the new version will use the hard disk. Also, since I don't follow MINIX as much as I use to or should since it was my intro into the unix world and I still read the manual sometimes, you should look at the newsgroup "comp.os.minix" or something to that effect.
