Peter da Silva sent me the answer by email, probably
to save me the public embarrassment, (thanks Peter) it
was booting as "sd(1,a)" instead of "hd(1,a)".
It's been a long time since I last used unix, I must
have forgotten you are allowed to think while installing
software.
