Matt Thomas wrote it for FreeBSD-1.1.5.1 
based on the PCI code in the NCR PCI SCSI
driver patch kit.

I don't have any 1.1.5.1 system here, but 
if you want to try it, just get and install

FTP.Uni-Koeln.DE:packages/FreeBSD/PCI-SCSI/scsi-ncr-941026.tar.gz

and copy the DE driver relevant files to
/sys/i386/pci (if_de.c and dc21040.h).

There my be some small modifications required,
but I still have revision 1.1 of the de driver
sources (as of October 1, 1994) on my system
to aid in porting  the most recent version of
the driver back to 1.1.5.1 ...
