I have a Diamond Stealth64 VRAM w/ 2MB and a NEC6FG 21" monitor.  My 
device section from /etc/XF86Config is:

The key was setting up the correct Ramdac and Clockchip as listed in the 
device configuration file (unfortunately, for the life of me, I can't 
remember its name).

This file saved me both on my DS64V and on an old Diamond Stealth Pro VLB 
on another machine.
