I didn't recompile mtools from scratch, but made the MAKEDEV fd0/fd1 in /dev and manually changed the permissions to 666 after I once logged in as "me", not as root, and encountered the error message. I haven't seen it again after that. BTW, there was some recent patch of fd.c by pa-ola@proxxi.se, which broke all, because it uses the fd minor number for the density. Although a patch for mtools is available, you won't be able to build a working dist.fs kernel without messing around with the /dev's around everywhere. I recommend not to apply this patch.
