Well, I have a setup similar to the one that you are describing:
OS FreeBSD
mail: mh & exmh
Zyxel U1496E  handles voice/fax
Ascend Pipeline 50 ISDN router for a dedicated 128kb ISDN line
P100 32MB 
Gravis Ultrasound Max

etc.... at least these are the relevant stuff.

For fax/voice mail, I use mgetty+sendfax . I can pickup my phone messages
remote using a phone, check my mail, or voice spool. Boot my machine 
remotely by calling my system and telling it too.

on how to build mgetty+sendfax.


I am pretty sure that FreeBSD, NetBSD or Linux will be able to meet
your requirements.
