I have experienced what may be a similar problem on my system.  I have a
Wangtek 5150EQ (QIC-02 controller) that appears to write tapes okay but
when I try to read back the tape (or a tape written by another system)
it appears to be missing the first 2KB of each block read.  (I was using
a buffer size of 2MB as I recall).  This failure seems to happen with
dd, cpio, tar.  Unfortunately, I have not had the time to work on the
problem since discovering the missing 2KB at the front of each block.
