I'll bet you're using a 3c509 in a box w/a Micronics PCI motherboard. Some of these reserve IRQ 15 for PCI cards. BSD/OS puts ISA 3c509 cards there by default. Rebuild your kernel and put the ef0 driver at a different irq (making sure its a free irq) or just use command line args to the kernel upon bootup to achieve the same effect. The lizzard book explains how to do this.
