Ah-ha!  Someone else with precisely the same problem I'm having.

I'm not having too many errors on the debug screen.  Only when I get to
your second error.

I get these exact errors.  With the exception that my drives are SCSI
drives.  In my other post I gave the relevant debug information.  I also
have a lot more of the debug info squirreled away somewhere.  It comes
down to newfs not being able to create a new filesystem I think.

My configuration when I started to install FBSD and when I finally gave up
I had removed one of the SCSI cards.

2 NCR 810 SCSI controllers
  0 - Quantum Atlas 2.1GB
      a) 200MB DOS partition
      b) linux
      c) linux swap
  1 - Quantum Grand Prix 4.3GB
      brand spankin' new
      A)ll configuration
         root, swap, var, usr

Hmm... maybe it has something to do with the name of the installer? :)
