What I have found out to be a *BIG* culprit of this problem is Internet In A Box's mail application. Every customer this happened to, had IIAB. IIAB addmitted that there was a bug that caused this and finally came out with an upgrade (2.0) for the mail and it has pretty much ceased. Sometimes a dropped modem connection during mail retrival can cuase this too.
