I have looked at USR standalones, USR rackmounts, Ascend Max 4000, Microcom, and Multi-Tech modems. I have considered Digital Channel Service for my incoming local calls as well as centrex-type service. I plan on starting with 24 - 48 modems and grow to 96 by year's end. My startup budget has ranged from $80 to $160k. I have been approached by other service providers who claim that they can provide a more efficient back room than I can.
