It turned out that the CD-ROM drive did not exist at its documented I/O location until after the driver for the sound card has been loaded by MS-DOS. To use the CD-ROM drive with FreeBSD, the following sequence should work: 1. Boot MS-DOS, let it load the driver for the sound card. 2. Hit alt-ctrl-del and now boot FreeBSD.
