I knew I'd forgotten something.  I'm running the XF86_SVGA server.
I can't remember everything I listed about my system last time, so to recap:

486DX2/50 w/ 8Mb
Cirrus Logic GD5428 w/1Mb
basic 101 keyboard
FreeBSD 2.1
Running xdm from rc.local with no command line options and no modification of
   any config files from the FreeBSD installation
Xserver is XF86_SVGA
Window manager is ctwm
