It was reported correctly at boot time. That one has AUI and I am not sure if this applies to this case, but I had (visually) the same problem (normal boot-init and 'ed0: device timeout' after ifconfig). I found that to use SMC EtherEZ card (8416BT) you have to completely turn off the PnP on it using text-mode EZsetup utility (not the fancy-GUI EZstart). Otherwise card did not work and its config was being screwed after each BSD run. Also, EZstart somehow failed to see the coax connector on the card.
