This sounds like dodgy hardware.  Do you see any errors during
installation ?  Check the diagnostic screen on ALT-F2.

If you see hardware errors, it could be due to bad termination etc. on
the scsi adapter.

Hmmm, I havn't convinced myself :(  Try another adapter ?
