Perms for /dev/mem:

crw-r-----  1 root  kmem    2,   0 Dec  6  1995 /dev/mem

(just like on our working BSD box..)

Perms for ps are correct too:

-r-xr-sr-x  1 bin  kmem  159744 Dec 17  1995 /bin/ps

The only other answer I've gotten was to mount /procfs, which has
been mounted.  Any more ideas?  Again, this system is running 
FreeBSD 2.1.5-RELEASE.
