Anything based on the Dec Tulip chip (21140 & related chips) is
excellent.  I've had success witht he SMC cards; I've heard good things
about the Kingston cards ($45 is the primary good thing) but haven't
tested them personally.

I can't vouch for *BSD with tehse cards, but I think it supports them 
very well, a friend looked at the driver and liked it.  I know they work 
well under Linux but that is probably of little concern here :-)
