Have you made any changes (adding/removing scsi stuff, changing cables, terminators?) If not, you might have a bad disk (or loose cable) Do you use other devices then your disk on that scsi-chain? I have seen simular stuff with defunct exbytes... (I like to use IDE for root, then i have "for free" a bootable disk where i might have logfiles etc).
