I  have an ISA bus SRS(mfr)  modem/midi/audio card which came installed with
the new Packard Bell E153 I just installed 2.1 on. It came with 
no useful documentation, has no occupied jumpers , and has as its
most prominant devices:

Rockwell RCV336ACF/ SP R6749-31 /9705 B28S62-5  68 pin package
AZTECH systems AZT2320
ISSI IS27HC010-45PL

If anyone has had luck getting 2.1 to recognize the modem on this card
during any of the boot- time probes for devices , a brief email response
would be appreciated. I have a feeling this board is destined for 
the spare parts bin.
(the unoccupied jumpers are not labeled.)
