I've just installed 2.2-RELEASE on a machine that is intended to be a
news server and have had one crash that looks somewhat similar:
Mar 21 13:07:42 news2 /kernel: sd0(ahc0:0:0): SCB 0x3 - timed out while idle, LA
STPHASE == 0x1, SCSISIGI == 0x0
Mar 21 13:07:42 news2 /kernel: SEQADDR == 0x8
Mar 21 13:07:42 news2 /kernel: Ordered Tag queued
Mar 21 13:07:42 news2 /kernel: Ordered Tag sent

I'm running a kernel with tagging and SCB paging enabled. I'm trying
to reproduce the crash now bnefore trying again without the fancy AHC
stuff.

The same machine was running 2.2-GAMMA for a while without hitting this
problem. (Though it had other disk related problems.)

The config is PPro 200, Adaptec 3940UW, Quantum Atlas II.

Any theories?
