I have "keymap=german.iso" in my /etc/sysconfig file.

I had this problem for some time, too.  I used xkeycaps to check
the settings and when I selected the PC102-german layout in it I
found that the umlaut-keys are recognized perfectly but at some
point that I haven't found they are remapped to produce different
glyphs.  So basically I reset the keymap to its default state and
then the umlaut keys work again.  Now I have a ~/.Xmodmap that
corrects the mappings automatically.
