Make sure your drives can actually handle the transfer speeds you have the SCSI controller set for. In all the cases where the drive didn't physically die, the failures I have encountered by having the transfer rates set too high for what the device can *really* handle, which may be lower than what it *claims* it can handle. The other frequent cause for strange errors is poor quality (and too long) SCSI cables, or cables that are made incorrectly.
