I'm getting repeated errors of the form:
sd10(ahc1:2:0): MEDIUM ERROR info:867e49 csi:16,98,4,6 asc:11,0 Unrecovered read error sks:80,80, FAILURE

The details are always the same.  The disk is a Quantum Atlas II.  According
to the mode page dumps, auto read and write reallocation is enabled.

Can anyone tell me how to decode this particular error?  I presume
the info is a block number in hex.

Also, is there a way to instruct the disk to replace the sector?  (I'm looking
for the equivalent of SunOS format's repair command.)
