Try using /sbin/diskdefect. (or at least reading the man page)

Used it to cure our 'wd0: error in bad sector file' - 

HOWEVER, our problem was due to installing onto a large EIDE drive and
not scanning it at install time i.e. there was no bad sector file, so
diskdefect was used to write a new one (although EIDE doesn't need a
bad sector file anyway, so the operation was purely cosmetic)

'FORMAT error in bad sector file' could be a 'proper' problem.
