I think it was not much more than configuring pcvt, with both the mono
and VGA card present.  I eventually forgot the details however.

No, pcvt will still manage /dev/ttyv*.  It will act as both, the mono
console driver, and the hookup point for the Xserver to handle the VGA
hardware (as well as the means to multiplex the keyboard).

By the time i did it, i've only been investigating for people running
a fixed-frequency monitor that couldn't display VGA.

Ah, i remember one detail: the BIOS had to be told to use the mono
screen as the display, i think i had to set a jumper on the mainboard
for this.  Quite possible that you'll be outta luck with modern
boards/BIOSes.
